ATTENTION: Local Social Security offices have closed their doors to the public.

How does this impact your client’s ability to sign up for Medicare?

Our suggestion:

1. If your client is starting Medicare at the age of 65, the best way to do this is to enroll online (easiest and fastest) or, call your local Social Security office. Please use this link to sign up online: https://www.ssa.gov/benefits/medicare/

2. If you need to enroll into Medicare’s Part B AFTER age 65, our recommendation is to do the following: Fill out the form 40B and form L564 (the employer verification form) and send them priority mail (make copies and obtain a tracking number) to your local social security office. Please use this link https://secure.ssa.gov/ICON/ to locate your local office. If you have any concerns about the best method for you, please call YOUR LOCAL OFFICE to ask them questions.

Please click on each link to access the forms: 40B and L564 forms that you need if you are signing up after age 65:
